First Visit

First Dental VisitWe would like to welcome you as a new patient to our office. We look forward to providing you with the quality dental care that you deserve.

 

Here is what you should know about your first visit:

 

We need to get to know you. That involves that dreaded paperwork. It will consist of a Patient Registration form, Medical and Dental History form, Financial/Cancellation Policy, and a Cosmetic Questionnaire. If you would like to take a few moments, you can locate these forms at our Patients tab, fill them out, print them and bring them with you.

 

Now for the dentistry… you will probably need dental x-rays. In order to get a full perspective of your mouth, we will need a full set. If you have had x-rays taken within the last year, please be sure to bring a copy with you or have your previous dentist send us a copy. Or, let us know, and we can request them for you. Along with x-rays you will have a comprehensive oral examination of your teeth and gums, looking for decay and other problems, unless you visit us for a specific concern.

 

Be ready to talk about any current dental problems you may be having. If this is a get-acquainted visit or are here for your regular cleaning, more power to you. However, if you have something bothering you, please let us know. We want to address this immediately. We are also happy to address any cosmetic dental concerns at this time.

 

Often your first visit will include a teeth cleaning. If it has been a while since you have had your teeth cleaned, you may need to be scheduled for another visit. The Doctor will discuss this with you.

 

Time to decide...The Doctor has done a complete exam, made a diagnosis and informed you of his findings. Now you need to ask any questions you have about treatment, so you can make an informed decision about going forward with any treatment that is needed.
